Laura Jurewicz, doctoral candidate at Midwestern University's Clinical Psychology program in Downers Grove, Illinois is currently writing her dissertation on the importance of death anxiety and its affect on medical professionals decision to work with an elderly population. She is asking physicians to assist in this work by completing this survey.

Title of Protocol:
The Importance of Death Anxiety and How it Affects Medical Professionals Decision to Work with an Elderly Population

I. PURPOSE OF THE STUDY
The purpose of this proposed dissertation is to examine how death anxiety affects osteopathic medicine physician's choices in treatment populations specifically its relationship to one’s choice to work the elderly.

II. PROCEDURES
You will be one of approximately 50 subjects to be asked to participate in this survey.
This survey will be provided to all participants through the AOA state appointed representatives. Each email will provide a link to surveymonkey.com to reach the assessments conveniently. This website is used to collect data while keeping the raw data completely anonymous. Each participant will be given 20 minutes for the assessments, however it will likely not require the whole 20 minutes to perform. The survey will then, be collected by the researchers and used to evaluate the purpose of the study.

III. RISKS/DISCOMFORTS
There are no anticipated risks to this study.

IV. BENEFITS
There are no direct benefits to the participants in taking this survey, however the results from the study may benefit the field of clinical psychology as well as osteopathic medicine. This study may provide insight into the shortage of osteopathic medicine physicians specializing in older adult care.

V. ALTERNATIVES

The survey is voluntary and those asked to participate may decline at any time throughout the survey.

VI. FINANCIAL COSTS TO SUBJECTS
There will be no financial cost to subjects.

VII. STATEMENT ON COMPENSATION FOR RESEARCH INJURY
Not Applicable

VIII. PAYMENTS TO PARTICIPANTS
Participants will receive no compensation for their participation in this study.

X. CONFIDENTIALITY
All data collected will be completely anonymous. No identifiable data will be collected.

XI. CONSENT:
By clicking on the provided link you are consenting to be a participant in this research study. This link will take you to the surveymonkey website where the confidential survey will appear. The survey will begin with a demographics page asking age, gender, race, length of practice, and medical specialty. You will then be asked questions about your level of death anxiety. Following the completion of the study all of the data collected will be kept in a storage cabinet in Department of Clinical Psychology at Midwestern University.
