The Patient-Centered Primary Care Home (PCPCH) Program has published a revised PCPCH 2017 Recognition Criteria Technical Specifications and Reporting Guide (TA Guide).

The 2017 PCPCH standards have not changed. The primary purpose for revising the TA Guide is to provide clarity on the technical specifications and documentation requirements for certain PCPCH standards and measures. Specifically, the guide includes:

  • Updated technical specifications for some PCPCH measures that align with the Centers for Medicare & Medicaid Services (CMS) renaming of the Electronic Health Record (EHR) Incentive Programs (Meaningful Use) to the Promoting Interoperability (PI) Programs.
  • The removal of two of the four measures from PCPCH Standard 2.E - Ambulatory Sensitive Utilization
  • Updated Consumer Assessment of Healthcare Providers and Systems (CAHPS) benchmark tables for Measure 1.A.3 and Measure 6.C.3

A full summary of the revisions is on the PCPCH program website - https://www.oregon.gov/oha/HPA/CSI-PCPCH/Documents/2017-PCPCH-TA-Guide-Sept-2018-Revisions-Summary.pdf
